Early Intervention Child & Family Connections (CFC) is a statewide system that has been established to ensure that all referrals of children under the age of three receive a timely response by an initial service coordinator in a prompt, professional and family-centered manner. There are 26 CFCs in the state.

The CFC #19 serves the following counties: Clark, Coles, Cumberland, DeWitt, Douglas, Edgar, Macon, Moultrie, Piatt and Shelby. This CFC is the central point of entry for all children needing Early Intervention Services. We provide intake and referral to community service providers.

Early Intervention Services are designed to help each child reach his or her individual potential. These services specifically address a child’s developmental delays or disabilities. Children who may be eligible for early intervention services have one or more of the following conditions:

Motor, intellectual, emotional or speech delays

Diagnosed disabilities such as Cerebral Palsy or Downs Syndrome
-
Conditions that put them at risk for developmental delays such as extremely premature birth.

Funding for these services is provided in full or in part by the Illinois Department of Human Services. Services are also provided without restriction as to race, creed, religion, sex or economic status.

What We Do
Fourteen different services are available through Early Intervention. All of these services are to promote and enhance development. The provided services are:

Service coordination

Developmental therapy

Psychological

Physical therapy

Occupational therapy

Speech and Language therapy

Audiology
-
Visual
-
Social work
-
Medical consultation
-
Nursing
-
Nutrition
-
Family support
-
Transportation

CFC #19 is responsible for:
​

Responding to all referrals within 48 hours
-
Giving families information about Early Intervention services
-
Determining eligibility – obtaining reports and evaluations that provide information to assist in eligibility determination.

Providing parents with information regarding their rights under the law and with the State of Illinois Infant\Toddler and Family Rights Handbook.

Offering parents the opportunity to choose service providers to perform evaluations and on-going services.

Working with parents and providers to complete the intake process and facilitate the writing of the individual family service plan (IFSP).

Continue to monitor services and locate additional resources as needed.

Offer Parent Liaison services to parents/guardians to provide families with additional support from someone who has a child with special needs and understands the process and feelings.

CFC works closely with local early intervention providers and agencies to identify ways to eliminate service gaps.

Make a Referral
Anyone can make a referral to the CFC. To Make a referral simply call 1-800-768-2990 or (217) 423-6199 and we will do the rest. You do not have to be certain there is a delay to refer a child. Any concern is worth looking into and there are no costs for evaluations.
Please be prepared to provide us with the following information:

Parent or Guardian's name, address, and telephone number

Suspected delays or reason for the referral
